How To Pronounce Guttate Psoriasis

What Does Psoriasis Look Like

How To Say Guttate

Psoriasis usually appears as red or pink plaques of raised, thick, scaly skin. However, it can also appear as small, flat bumps or large, thick plaques. It most commonly affects the skin on the elbows, knees, and scalp, though it can appear anywhere on the body. The following slides will review some of the different types of psoriasis.

These Are The Most Common Psoriasis Symptoms:

The most common type of psoriasis is plaque psoriasis, per the Mayo Clinic. The symptoms of plaque psoriasis are most often found on the scalp, elbows, knees, lower back, and genitals and may include:

  • Raised, red patches of skin
  • A silvery white coating of dead skin cells on top of those patches
  • Itchiness
  • Pain
  • Cracking skin and bleeding

There are other, less common forms of psoriasis. Guttate psoriasis, which often begins in childhood or young adulthood and can be triggered by a strep infection, appears as small, droplike lesions. Inverse psoriasis shows up as a smooth, shiny, red rash in body folds, such as under the arms or breasts. Pustular psoriasis, which can present on the palms of the hands and soles of the feet, is characterized by noninfectious, pus-filled blisters, according to John Hopkins Medicine. The most severe, and rarest, type of psoriasis is erythrodermic psoriasis, which results in widespread, fiery redness over most of the body. It can cause severe itching and pain, and if you think youre having an erythrodermic psoriasis flare-up, you should seek medical attention immediately.

What Causes Psoriasis Vs Eczema

These two skin problems have different causes and can appear in different stages of your life. Psoriasis is an autoimmune condition that affects about 7.5 million Americans, according to the AAD. It happens when the skin cells go through their life cycle more quickly than normal. Typically, it takes about a month for skin cells to regenerate, but in people with psoriasis, this process happens every three to four days, according to the Cleveland Clinic.

Psoriasis typically strikes when someone is between the ages of 15 and 35, according to the U.S. National Library of Medicine, but it can affect anyone at any age. What we do know is that psoriasis is immune-mediated, meaning there is some imbalance in the immune system, which leads to inflamed skin, dermatologist and National Psoriasis Foundation medical board member Jashin Wu, M.D., tells SELF. It tends to run in families, but there may not be straightforward patterns of inheritance. One well-known trigger is strep throat infections, but not all new cases of psoriasis result from strep throat.

Eczema is much more common than psoriasis. According to the most recent estimate available, more than 30 million Americans have some form of eczema.

Three Ways To Deal With Psoriasis

Medical Matters/Dr Muiris Houston: A recent letter from a psoriasis sufferer had this to say: “I’m more or less living with my psoriasis, but yesterday on the train to Dublin I met a young woman on her way to hospital . . . . Her fare to Dublin is costing her 30 each time she goes for treatment. If she could get into she would be spared that cost.”

Asking if I could highlight the 90 the young woman has to spend on transport for a typical week’s treatment, she asks: “Could you tell the Minister for Health how much it annoys us to hear him say, when challenged, ‘I’ve given X billion this year,’ when what he should be saying and doing is concentrating on people and how to improve life for them?”

It is a well-made point, although of the recent crop of health ministers, Micheál Martin is generally regarded as a most caring man. The problems we now see in the health service are the result of years of chronic underfunding. It will take 10 more of sustained effort, money and realignment to make the system more inclusive and caring. Unfortunately, at the last Budget, the additional funding that had begun to flow was abruptly cut off.

The condition causes dull, red and scaly plaques on the skin. It typically affects the front of the knees and the back of the elbows. The sacrum – the lower back below the lumbar area – and the scalp are other common locations. Referred to as chronic plaque psoriasis, its salmon-pink colour and scaly appearance are unmistakable.

Can Stress Exacerbate Psoriasis Or Eczema

Theres no doubt that living with psoriasis and eczema can be stressful, and their connection to stress has been widely discussed in the dermatology community.

Although experts often advise people with psoriasis and eczema to avoid stressful situations, a lot remains to be proven about exactly how stress can influence these conditions. A November 2017 meta-analysis in the British Journal of Dermatology evaluated 39 studies with more than 32,500 patients, concluding that no convincing evidence exists that preceding stress is strongly associated with psoriasis exacerbation/onset. As for eczema, an October 2017 study in the International Journal of Molecular Sciences reported that psychological stress may exacerbate atopic dermatitis, and it might be mediated by the hypothalamic-pituitary-adrenal axis.

Just because science hasnt fully clarified the role stress plays in these conditions doesnt mean the link doesnt exist. Dr. Wu says hes seen patients experience stress-related psoriasis or eczema flare-ups in the past, and the AAD mentions stress when discussing both psoriasis and eczema.

How The Intervention Might Work

The link between psoriasis and streptococcal infection is probably explained by the ‘superantigen theory’. The streptococcus carries a protein called the Mprotein which allows it to act as a superantigen. Superantigens are bacterial or viral products that can bypass normal immunological pathways and cause powerful stimulation of the immune system. This results in the production of Tlymphocytes which have been shown to be central to the development of psoriasis .

Although there is good evidence that streptococcal infection is important in psoriasis, some dermatologists believe that a wider range of organisms may be implicated in exacerbating the disease. Skinner and colleagues have suggested that the activity of psoriasis is always the result of microbial deposition in the skin . They have also suggested that infection or merely colonisation by microbial organisms as diverse as the yeasts Candida albicans and Malassezia furfur, colonic bacteria such as Enterococcus faecalis, Escherichia coli, Pseudomonas and Proteus species, or the bacterium implicated in duodenal ulceration, Helicobacter pylori, may also be important. They suggest that treatment with appropriate antimicrobial agents should be given and that 60% of patients will improve with such treatment . The studies in this review were, however, uncontrolled and it would appear that no attempt was made to quantify the response.

Antibiotics Or Tonsillectomy For People With Psoriasis

Psoriasis, a disease which produces scaly pink patches on the skin, often gets worse after a bacterial throat infection or tonsillitis. When someone with psoriasis has a throat infection, antibiotics are sometimes used in an attempt to prevent this from happening. For people with psoriasis and repeated throat infections tonsillectomy is sometimes recommended. The review found no evidence from trials that either antibiotics or tonsillectomy are helpful for people with psoriasis.

Why It Is Important To Do This Review

Guttate psoriasis, guttate psoriasis is a type of ...

The mainstays of treatment for both guttate and mildtomoderate chronic plaque psoriasis comprise topical therapy with tar, anthralin , vitamin D analogues or corticosteroids, often combined with phototherapy. Whether administration of antistreptococcal antibiotics can alter the course of guttate psoriasis or chronic plaque psoriasis remains uncertain. This review aimed to assess whether there is a place for antistreptococcal antibiotic therapy in psoriasis and, if so, whether subgroups of patients who may benefit can be identified.

This review also examined whether there is any evidence which would support the use of tonsillectomy for selected patients with psoriasis.

The treatment of guttate psoriasis is addressed in a companion Cochrane review .

Description Of The Condition

Chronic plaque psoriasis is a condition affecting approximately 2% of the population in temperate zones of the world . Typically, red patches covered with silvery scales develop in areas such as the scalp, knees and elbows. In severe cases psoriasis may involve the majority of the skin surface. The factors which trigger its onset are poorly understood but there is good evidence that streptococcal infection, particularly tonsillitis or streptococcal sore throat, may play a role in initiating, perpetuating or exacerbating the disease .

Guttate psoriasis is a distinctive form of psoriasis which characteristically occurs in children and young adults. It is strongly associated with preceding or concurrent streptococcal infection . Typically, showers of tiny erythematous papules erupt explosively over large areas of the skin surface one to two weeks after an episode of acute tonsillitis or pharyngitis . In the early stages before the typical scale has had a chance to develop, guttate psoriasis may be mistaken for a drug eruption, especially in those patients given antibiotics for an associated streptococcal infection. The true diagnosis, which is a clinical one, soon becomes apparent as characteristic psoriatic scaling develops on the surface of the papules.
