It is not uncommon to have psoriatic plaques in areas where clothes are tight, causing friction to the skin. This picture is one such example in which pressure from a belt can give rise to a ring of psoriasis around the waist.

The phenomenon, known as the Koebner response, is associated with psoriasis and other diseases in which a rash or lesions will develop along the line of trauma.

With psoriasis, a flare may occur as a result of a cut, abrasion, sunburn, rash, or even vigorous scratching. It is even possible for psoriasis to develop on old scar tissues, including tattoos and surgical scars.

In this photo, the plaques have begun to heal. Once this is complete, the skin will often look lighter or darker than the surrounding skin. In most cases, the discoloration will normalize over time.

Can There Be Complications From Guttate Psoriasis

As with any type of psoriasis, guttate psoriasis may lead to complications.

Some people can have one episode of guttate psoriasis when they are 12 years old and never have it again, Sugarman says. Others wind up with chronic psoriasis and some lipid abnormalities: high blood cholesterol and triglycerides.

The gluteal cleft is a common site for plaque psoriasis. This photo illustrates the characteristic plaques with their clearly defined borders and patchy scales.

The scales are one of the clues that differentiate plaque psoriasis from inverse psoriasis, which can also develop in the gluteal crease. However, with inverse psoriasis, the lesions tend to be smooth with few visible scales.

Inverse psoriasis is more common in people with obesity who have more skin folds and greater amounts of adipose tissues.

The problem with gluteal involvement is that the very act of sitting can irritate inflamed skin and make the condition worse.

The exact pathophysiologic mechanism in guttate psoriasis is undetermined. Guttate psoriasis is believed to result from an immune reaction triggered by a previous streptococcal infection in a genetically susceptible host.

Studies indicate the importance of chromosome 6 in determining the resultant psoriatic phenotype. HLA-Cw*0602positive patients are more prone to develop the guttate form. Interactions of HLA-C with killer immunoglobulinlike receptors on natural killer cells or natural killer T cells can be deregulated by streptococcal infection. T lymphocytes and cytokines are believed to cause the characteristic inflammatory changes appreciated on histopathologic examination of lesions.

An autoimmune phenomenon has also been postulated to underlie guttate psoriasis because some streptococcal products and components have been found to cross-react with normal human epidermis. Electron microscopic studies of guttate psoriasis have shown that mast cell degranulation is an early and constant feature in the evolution of guttate psoriatic lesions. Furthermore, Langerhans cell migration appears to be impaired during an acute episode of guttate psoriasis.

    The nosologic relationship of psoriasis and tattoo remains a matter of debate. Despite the intuitive concern that tattoo trauma might induce the disease , a KP was reported in only 3.2% of psoriasis patients undergoing tattooing , an overall rather moderate rate . Moreover, Grodner and colleagues observed that much more patients with psoriasis did not recall lesions that had affected their tattoo sites during episodes of disease flare-ups than recalling them . Herein, on the occasion of a patient who presented with a distinct sparing of three tattoo sites during a guttate psoriasis episode that was unrelated to tattooing, we would like to comment on the putative role of macrophages as the key cellular link in the complex pathophysiologic relationship between tattooing/tattoo and psoriasis.

    Sara Spencer : I have guttate psoriasis! At the beginning of a flare right now. Strep/tonsilitis flares mine. I had it strep about 3 weeks ago and theyre starting to pop up. This is my third flare, goodish news, all of my flares have lasted a year tops, most done around 10 months! I tried everything but biologics because of insurance, and not much had worked for me.

    It sucks, but changing diet gluten, dairy and ESPECIALLY ALCOHOL are all cut out. I notice a big difference without alcohol. So I use a a liquid for your scalp and mix it into OTC lotion and that helps keep the scales down and from it being painful etc. I also exfoliate every time I shower with an exfoliating sponge. Im going to try tanning for 2 minutes a session a couple times next week and see how that goes!
