Pathophysiology Of Guttate Psoriasis

The exact pathophysiologic mechanism in guttate psoriasis is undetermined. Guttate psoriasis is believed to result from an immune reaction triggered by a previous streptococcal infection in a genetically susceptible host.

Studies indicate the importance of chromosome 6 in determining the resultant psoriatic phenotype. HLA-Cw*0602positive patients are more prone to develop the guttate form. Interactions of HLA-C with killer immunoglobulinlike receptors on natural killer cells or natural killer T cells can be deregulated by streptococcal infection. T lymphocytes and cytokines are believed to cause the characteristic inflammatory changes appreciated on histopathologic examination of lesions.

Electron microscopic studies of guttate psoriasis have shown that mast cell degranulation is an early and constant feature in the evolution of guttate psoriatic lesions. Furthermore, Langerhans cell migration appears to be impaired during an acute episode of guttate psoriasis.

It Often Comes Hand In Hand With Other Illnesses

Guttate psoriasis can commonly be linked to upper respiratory infections , particularly streptococcal infections, says Mohiba Tareen, MD, a dermatologist at Tareen Dermatology. Sixty-six percent of those with an acute guttate psoriasis flare had a clear history of a recent severe URI. The science shows that strep infections can activate immune cells called T lymphocytes that then turn on psoriasis in genetically predisposed individuals. Generally, the rash appears a few weeks after the illness that caused it subsides. Even so, you should learn these strep symptoms so you dont avoid a URI diagnosis and delay treatment.

My mom is having what her doctor told me is a guttate psoriasis flare-up after we had to bring her to the hospital since her throat infection didn’t seem to go away even after the antibiotics. She has psoriasis for the last 10 years and she’s had bad flare-ups, but honestly, this one seems far more dangerous. She is now constantly on the steroids and will have to go for UV light therapy for the most part of the week, but none of this is helping to get this a guttate psoriasis flare-up into remission. If anybody can help me understand – how long does guttate psoriasis last and will these flare-ups come back this strong? Will all this treatment have any effect at all or will she need to do this over and over again?
