Insomnia And Psoriasis: A Vicious Cycle
Like most people, I dont feel as productive or focused after a night of broken sleep. Making things even worse is that lack of sleep is a trigger for my psoriasis and eczema. I feel trapped in that vicious cycle of wakefulness that sparks skin inflammation, and vice versa.
In doing a bit of reading online I found that Im not alone in the battle for a good nights sleep, and that more than 85 percent of people with psoriasis struggle with this issue.
Researchers at the University of California in San Francisco surveyed more than 3,000 patients through the National Psoriasis Foundations Citizen Pscientist online forum and found that the more severe the psoriasis, the greater the sleep disruption and the poorer the sleep quality. They published their findings in the Swiss journal Dermatology and Therapy in September 2019.
A study published in April 2018 in the British Journal of Dermatology further confirmed a link between psoriasis and sleep problems. The authors looked at almost 300 people with and without psoriasis and found that 25 percent of those with psoriasis reported clinical insomnia, versus 10.5 percent of those without psoriasis. In addition, almost 54 percent of those with psoriasis were poor sleepers, versus close to 22 percent of those without psoriasis.
The authors cited itch as the main predictor of impaired sleep, and suggested that improved control of psoriasis with decreased itch may improve sleep disturbance in psoriasis.
Slather On A Soothing Moisturizer
Roberson also recommends applying products that are calming and restorative before bedtime. Whatever brand you choose, the American Academy of Dermatology suggests selecting something that comes in a heavy cream, ointment, or oil, rather than a lotion, and making sure the product is fragrance-free. Besides reducing itch, Ive developed other habits to improve my sleep quality. One major sleep disturbance for me is stress. If Im experiencing a lot of conflict at home or at work, am behind on deadlines, worrying about my health, or making a major life decision, then my mind wont rest.
During stressful times, like the ongoing pandemic, I need to practice effective ways to address the underlying issue, such as talking things out with my wife or quieting down in prayer and meditation.
Ive learned not to start a project or reply to emails too late in the evening, as this overstimulates my mind when I need to start relaxing.

Another good habit for better sleep is to start routines early. For instance, I prefer to exercise after work and even later in the evening when its cooler, but going on a run then tends to wake me up. My skin-care regimen takes time, so if I start off by showering late it throws my sleep schedule off.
I love coffee and have made it a hobby in recent years, roasting, grinding, and brewing espresso with beans I purchase online. But drinking coffee after the early afternoon is a recipe for a long night of wakefulness. Saying no to that evening cup of coffee is a choice for better sleep.
I do like naps after a long day at work, but since it can lead to insomnia, I do my best to resist.
Why You Have Itchy Skin At Night
It’s not entirely clear why nocturnal pruritus does its thing at the most inconvenient time ever, but doctors do have a few theories. “There are many physiological changes that occur in your body at night, including changes in thermoregulation, skin barrier function, and fluid balance,” says Donna Hart, M.D., a dermatologist at Westlake Dermatology in Austin, Texas.
For starters, your body regulates its core temperature to be lowest at night. It does this by increasing blood flow to the skin, which is what allows the heat in your body to dissipate, says Dr. Hart. This temporary uptick in body temperature may correspond to increased sensations of itchiness.
Your skin’s ability to retain moisture is also lowest at night. Not only can the subsequent dryness cause itchy skin, but an impaired skin barrier makes it easier for itch-inducing substances to cause drama, says Dr. Hart.
These substances are produced by your cells for a variety of reasons, such as managing inflammation , and can be triggered by your environment as well . Once the substances are released, they bind to specific receptors that activate nerve endings in the skin, triggering itchy skin at night and frankly any time.
To top it off, the body’s anti-inflammatory corticosteroid levels are lowered at night, while certain itch-inducing hormones and cytokines are released in higher quantities a one-two punch that can make the sensation of itchiness that much more noticeable.

Stress Anxiety And Depression Can Make Itchy Skin Worse At Night
There’s a strong association between psychological issues and nocturnal pruritus and itching at night, though it’s not clear how one leads to the other, says Dr. Hart.
One theory is that psychological drama can hyper-activate the body’s fight-or-flight response, flooding the body with hormones and activating the immune system as protection.
When stress happens in small doses, the body’s release of cortisol has an anti-inflammatory effect that may cancel out the stress-induced itchiness caused by the excess histamines. But chronic stress and anxiety can interfere with your body’s ability to produce enough cortisol to pick up the slack , says Dr. Hart, and this may be what makes the itchy skin at night that much more intense when you’re always stressed out.
Another possibility: Because our skin is a direct pathway to our nerves, and our nerve endings go into overdrive during psychological stress, this can also result in intense itching, says Michele Green, M.D., a New York-based dermatologist.
A cycle of sleep deprivation and itchiness may follow, since an out-of-whack sleep cycle can dysregulate the hormones necessary to keep nocturnal pruritus in check, resulting in a cascade effect that ultimately triggers you guessed it itchy skin at night only.
Worse Itching Means Worse Sleep
Overall, researchers found that 59 percent of participants with psoriasis reported problems sleeping, compared to 34 percent of those without psoriasis. The study also found that people with psoriasis got a median of six hours sleep per night versus seven hours for those without psoriasis. The Centers for Disease Control and Prevention recommends adults get at least seven hours of sleep each night to maintain good health.
Perhaps unsurprisingly, researchers found that people living with psoriasis who didnt experience itching slept better than those who had strong or very strong itching. People whose itching flared up at night had the worst sleep quality.
