Coal Tar Treatment For Psoriasis

Original Articleno Increased Risk Of Cancer After Coal Tar Treatment In Patients With Psoriasis Or Eczema

Psoriasis Skin Care: Coal Tar Helped me A LOT for my psoriasis

Coal tar is an effective treatment for psoriasis and eczema, but it contains several carcinogenic compounds. Occupational and animal studies have shown an increased risk of cancer after exposure to coal tar. Many dermatologists have abandoned this treatment for safety reasons, although the risk of cancer after coal tar in dermatological practice is unclear. This large cohort study included 13,200 patients with psoriasis and eczema. Information on skin disease and treatment, risk factors, and cancer occurrence was retrieved from medical files, questionnaires, and medical registries. Proportional hazards regression was used to evaluate differences in cancer risk by treatment modality. Patients treated with coal tar were compared with a reference category of patients treated with dermatocorticosteroids . The median exposure to coal tar ointments was 6 months . Coal tar did not increase the risk of non-skin malignancies 0.92 95% confidence interval 0.781.09), or the risk of skin cancer . This study has sufficient power to show that coal tar treatment is not associated with an increased risk of cancer. These results indicate that coal tar can be maintained as a safe treatment in dermatological practice.

One success story:This has helped my psoriasis on my scalp more than I ever hoped. I was worried it would dry my curls out too much and I would be left with unmanageable frizz. Instead of a frizzy mess, my curls were left hydrated and I now have good hair days everyday! The scale has decreased dramatically, the itchiness has completely subsided, and spots are decreasing slowly in size.

Coal Tar Shampoos: The Good The Bad And The Smelly

Several of our readers have mentioned that they use coal tar shampoo as an over-the-counter treatment to control their scalp psoriasis, scalp eczema and dandruff. Coal tar is one of the few old-fashioned psoriasis treatments that has stood the test of time. While many people have seen positive results from coal tar shampoos, there is also worry about the long-term side effects and unpleasant scent of coal tar. Are coal tar shampoos for you?

Here are some of the benefits and risks associated with coal tar treatments:

The Good

Coal tar products are regulated by the FDA and are a recognized treatment for psoriasis. According to the National Psoriasis Foundation, Tar can help slow the rapid growth of skin cells and restore the skins appearance. In addition, it can help reduce the inflammation, itching and scaling of psoriasis. Coal tar shampoo is a keratolytic. It works by slowing bacterial growth and loosening and softening scales and crust. While the concentration of tar varies from product to product, the greater the concentration of tar, the more effective the shampoo.

The Bad

As with most treatments, the use of coal tar does not come without potential risks and side effects. According to the NPF, coal tar may irritate, redden or dry the skin. It also makes the skin more sensitive to sunlight. However, use of sunscreen and avoiding the sun within 24 hours of using coal tar products may help control sun exposure risks.

Tough to say, since tar soaps and other tar products come in different formulations. Most commonly these products are sold with 110 percent coal or pine tar.

Youll need to chat with your doctor and do some experimentation to find the right fit, but they can be bought over-the-counter as:

  • Bar soap. Best for treating psoriasis over large areas of your body. You use it a lot like regular bar soap, letting it sit on your skin before rinsing off for the best results.
  • Cream. Good for psoriasis on parts of your body other than your scalp. Generally, you apply a small amount of cream to the affected areas on your skin, one to four times per day for several weeks. Always check the product directions before applying.
  • Shampoo. Ideal for scalp psoriasis. You can use tar soap shampoo a lot like your regular shampoo, scrubbing into your wet scalp in the shower and then letting it sit before rinsing off.
  • Oil. Usually more concentrated than creams and shampoos. Tar oil is very quickly absorbed into the skin and is best suited for aggressive cases of psoriasis.

Stronger concentrations of tar soap, for very serious cases of psoriasis, are sold by prescription only. Again, your doc will know whats best and can help you craft the right treatment plan.

Most shampoos are designed to clean and care for your hair, but if you have psoriasis on your scalp, you should find shampoos that are made specifically for your scalp. These shampoos can help with flaking, redness, and itching. In some cases, that may be all you need to get a handle on it.

Often the shampoos have some of the same ingredients as the medicines and lotions your doctor might recommend for psoriasis on other parts of your body. You can get ones that treat mild to moderate psoriasis off the shelf at your local pharmacy. The two main ingredients in those are coal tar and salicylic acid. If you need something stronger, your doctor can prescribe it for you.

Whether prescription or over-the-counter, these shampoos should be used only for short periods of time. And youll need to be gentle with them. Your psoriasis can get worse if you scrub too hard. Its also important to follow your doctors instructions.

How We Chose The Winners

All our reviewers are experts in product formulation, haircare, qualified pharmacists or haircare experts.

They assess the formulation of the shampoo, deconstructing the shampoos to their constituent parts and describe the key ones in detail. In addition, they highlight the presence of any one of over 100 ingredients that can potentially itch, irritate and exasperate your condition.

Unlike most review sites, our reviewers personally try every shampoo they review and they also assess how the shampoo washes, how it smells and look at the price point in their market. These may be medicated shampoos, but that doesnt mean we want to compromise on a little bit of luxury!

We applied these same principles when choosing our best coal tar shampoo. For example, the winner is one of the gentlest dandruff shampoos weve reviewed. Its not perfect, but compared with the others it washed slightly better and the fact that it was fragrance-free gave it another edge. Thats not to say its the best coal tar shampoo for you. For example, Scalp 18 shampoo contains slightly more coal tar but is fragrance heavy. Theyre both outstanding shampoos, so we eventually chose the gentlest formulation.

How Do Coal Tar Shampoos Work

How coal tar reduces symptoms of psoriasis and seborrheic dermatitis is not completely understood.

Dr. Jaliman goes on, coal tar causes the skin to shed dead cells from its top layer and slow down the growth of skin cells. This lessens the dryness of the scalp and helps with dandruff.

Coal tar slows the growth of skin cells as it is keratolytic.

Its for this reason that Radha Mikkilineni, M.D., board-certified dermatologist at Sadick Dermatology, NYC, usually reserves use of coal tar for recalcitrant cases. But it acts by reducing the thickening of the scalp in a poorly understood manner whereby the keratinization and differentiation of the scalp skin is suppressed.

In addition to being keratolytic, coal tar has proven anti-fungal properties, antiseptic properties, anti-inflammatory properties and it also seems to normalize keratin growth on the skin and reduce the formation of scales.

What Is A Coal Tar Shampoo

A coal tar shampoo is a shampoo with up to 5% coal tar added as an active ingredient. Its is a much loved, much-used treatment that has been used medicinally for thousands of years. Despite its usage, theres still an air of mystery with this ingredient. For example, it contains over 10,000 chemicals , of which only 50% have been identified .

That said, its such an important healthcare treatment that:

Board certified dermatologist, Debra Jaliman MD, is a fan. She explained to us, coal tar can give you relief from itching and dry scalp. Coal tar extract helps with the winter dry scalp and it continues to work even hours after you have rinsed it off. It also helps who trouble with psoriasis and seborrheic dermatitis of the scalp.

What Is In Coal Tar

Coal Tar Cream

Coal tar doesnt have a definitive composition. Every batch has a slightly different formulation, based on the type of coal, temperature, distillation, and many other factors. This is unlike most other modern medicines and seems contrary to evidence-based medicine how can a product that cant be replicated be reliably studied?

What we do know is that crude coal tar contains over 10,000 compounds. This is then refined further into a solution to remove any harmful products, for example, ammonia-derivatives. The resulting mixture is then formulated into creams, ointments, soaps, or shampoos . At this stage other active ingredients can be added :

  • Calamine
Counteracting the bodys inflammatory response can treat many skin conditions and improve symptoms.
UV-radiation improves psoriasis. Coal tar sensitizes the skin to light, making therapy more effective than either used alone.

We asked Dr. Ben Barankin, a Toronto based board-certified dermatologist at the Toronto Dermatology Centre, for his advice, and he told us, For mild-moderate dandruff, I recommend daily shampooing, and ideally alternating 3 shampoos, each with active anti-dandruff ingredients.

Dr. Matthew Zirwas, board-certified dermatologist and founder of Bexley Dermatology in Bexley, Ohio, recommended trying a shampoo for, at least a month because if your old shampoo had ingredients that were causing a dandruff-like rash of their own, it will take 4-8 weeks for that ingredient to work its way out of your body after you start the new stuff.
