Effects On Other Treatments

You may be taking other disease-modifying drugs as well as apremilast. However, apremilast can interact with some medicines, so check with your doctor or a pharmacist before starting any new medicines. Remember to mention you’re on apremilast if youre treated by anyone other than your usual rheumatology team.

You can carry on taking non-steroidal anti-inflammatory drugs or painkillers, if needed, unless your doctor tells you otherwise.

Don’t take over-the-counter or herbal medicines without discussing it first with your doctor. Some of these, including St Johns Wort, can interfere with the treatment.

Is There A Vaccine Against Shingles

Shingix is currently the only shingles vaccine available in the United States. Its given to people over age 50.

Previously, an additional vaccine, Zostavax, was used, but it was phased out in the United States as of November 2020.

According to the CDC, two doses of Shingrix are over 90 percent effective at preventing shingles. Youll retain at least 85 percent protection for 4 years after being vaccinated.

If you get shingles after being vaccinated, your symptoms will likely be less severe. Youll also have a lower chance of developing postherpetic neuralgia a complication where pain remains even after a shingles rash goes away.

Shingles usually follows a pattern of development. It typically progresses with the following symptoms:

  • First, you may notice a tingling or burning sensation in your skin.
  • One to 5 days later a rash appears as small red spots.
  • Fluid-filled blisters develop a few days later.
  • After 7 to 10 days, the lesions crust over.
  • The rash disappears over the next 2 to 4 weeks.

In some cases, pain may persist for several months or even years after the rash has disappeared. This complication, known as postherpetic neuralgia , can be severe enough to affect your quality of life.

Certain antiseizure medications can help manage the pain. Gabapentin and pregabalin are two that are commonly used.

Shingles Dangerous For People With Autoimmune Disease

“Shingles — herpes zoster — causes blistering rashes around the trunk that last 10 to 14 days,” Curtis says. “But 20% of people have chronic and lifelong pain.”

Shingles occurs when chickenpox virus lying dormant at nerve roots reactivates, travels up the nerves, and causes painful blisters at the nerve endings in the skin. Anyone who has had chickenpox can get shingles, but people with weakened immune systems are most at risk. The CDC recommends a single dose of the shingles vaccine for people aged 60 and older.

But the FDA and other medical groups recommend against giving the vaccine to people taking biologic drugs such as TNF-blockers and other medications that suppress the immune system, including steroids. Medications that suppress the immune system can help people with autoimmune conditions, but they also leave them more vulnerable to infections — including infections from live-virus vaccines that are safe in people with normal immunity.

David Pisetsky, MD, PhD, says he would consider vaccinating people on biologic drugs against shingles. He is the chief of rheumatology at Duke University Medical Center in Durham, N.C.

“We should relook at this question,” he says. “TNF-blockers will be mainstays of therapy for some time to come, and we need to ask questions like, ‘What happens if you vaccinate?'”

    Am recovering from psoriasis and think I should get shingles vaccine but am scared to as afraid will renew psoriasis all over again.

    When I was around 49, I had the first singles vaccine. Around that time I was diagnosed with psoriasis. I had never had skin issues in my life. I used topical medications for a year and then read up on psoriasis chat sites and removed gluten from my diet and my psoriasis cleared up and has been under control for 10 years. In Dec 2019 I had the first installment of the shingrex vaccine, had flu like symptoms for 1-2 days, but all good otherwise. In mid July 2020, I, some family and neighbors had a chigger bite outbreak that I eventually needed a steroid cream for. At the end of July, I had the second shingrex shot, was flu sick for longer , had more reaction in my arm site and tired for a week. Shortly after, I started having a psoriasis outbreak on my left elbow, back hairline and on my back near spine and it has been going on for about a month now. Putting it all together now after 10 years, it seems obvious to me that there is a link.

    I’m sorry but I really have to do this. Why would it even occur to you to wonder if a vaccine for something totally unrelated would flare your psoriasis? No, it wouldn’t flare anything.
