Cool Or Warm Compresses

Some people might experience temporary relief from the itching by using cold compresses and taking cool showers.

Others also find that warm compresses ease the pain, so alternating between warm and cold compresses may help with both itching and discomfort.

People should not take hot baths and try to avoid being in the shower for longer than 10 minutes, as this can increase irritation. Applying lotion after a cool bath can help a person retain moisture in the skin, reducing the risk of dryness and itching.

Types Of Uv Light Treatment For Psoriasis

Meet UVB sun lamp the state of art device for psoriasis ...

Your doctor may suggest one or more of the following UV phototherapy or combination therapy approaches:

  • Ultraviolet A One of the most common options is short bursts of UVA, typically starting at 30 seconds in length and building in duration as a patient adapts to the exposure. A conventional treatment schedule calls for three sessions per week for 4 to 50 sessions, until the psoriasis has cleared. We typically go for 20 sessions at first, says Dr. Miller. If we dont see anything at all after 20 sessions, we rethink the treatment.
  • Ultraviolet B The Mayo Clinic notes that coal tar treatment makes skin more receptive to light, so some doctors combine UVB and coal tar treatments for a more effective therapy. The combination once required a three-week hospital stay, but a modification can be performed in a doctorâs office, according to Mayo.
  • PUVA For advanced cases of psoriasis, a physician might suggest augmenting UVA treatment with an oral medication called psoralen. Taken shortly before light treatment, psoralen seems to boost the therapeutic effect of light on the immune system. PUVA is spectacularly effective, says Miller. Because PUVA can be so powerful, the treatment is rarely used in children.

How Does Phototherapy Work

The exact cause of psoriasis is not fully understood and the effects of UV on the skin are complicated, so a precise explanation of how phototherapy works is not possible. PUVA and UVB phototherapy may also work in slightly different ways. However, a simplified description of the mechanisms of phototherapy will help to explain some of its side effects and restrictions.

Taking a simplified model of psoriasis as an example, the too-sensitive skin immune system causes localised patches of inflammation and the overproduction of skin cells, in turn causing the visible plaques. Phototherapy stops the overproduction of skin cells by either damaging their DNA or by preventing the cells from dividing by locking the DNA . It also suppresses the skin immune system to stop the psoriasis process. The twin processes of interfering with DNA and suppressing the immune system in the skin can also cause skin cancer, so phototherapy can increase cancer risk. Therefore, to safely benefit from UV phototherapy it is best administered under professional medical supervision.

During treatment, goggles must be worn to protect the eyes and most people also wear a clear UV-blocking visor to protect the skin of the face . Men must wear genital protection.

Safety Of Home Phototherapy

Treat Psoriasis with Phototherapy

Burning is more likely in fair-skinned people with Fitzpatrick skin types 1 and 2 compared to darker skinner people with Fitzpatrick skin types 4, 5 or 6. Burning is also more likely in people taking certain including common antibiotics and anti-inflammatory medications. Tell your physician the names of all your prescribed and over-the-counter medicines before you start treatment, and check with your doctor if you start a new medication while undergoing home phototherapy.

Home therapy can be safe if dosing is carefully controlled. Side effects reported by one study included red skin , blisters , itch , and dryness .

Safety features in newer home phototherapy units include:

  • a key-locked ON/OFF switch to prevent unauthorised use
  • a built-in controlled prescription timer that ensures that the equipment can only be used for a certain number of treatments as prescribed by the doctor between office visits
  • a fail-safe feature that disables the unit in the event of malfunction.

Such innovations make home phototherapy an appropriate option for many patients. Response to treatment and adverse events must be monitored carefully by your doctor.

Does Normal Sunlight Help

Regular sunlight has been used for centuries to treat psoriasis and other skin conditions. Some people suggest using a sunblock on unaffected areas then covering the affected areas about 10 or 15 minutes after youve been in the sun.

You just have to be careful since sunburn can actually have the opposite effect on psoriasis and actually make it worse. Short exposure times are key.

Make sure you talk to your doctor before you decide to try sun exposure just to make sure its right for you.

How Is Uvb Phototherapy Administered

There is no nationally agreed way to give a course of UVB phototherapy and different centres may use slightly different treatment plans. However, it is usually given three times a week for 6-8 weeks or until the psoriasis has cleared to an acceptable amount.

Many centres will check the UV sensitivity of the persons skin prior to a course of UVB phototherapy by applying a range of 6-10 different doses of UVB to small areas of normally unexposed skin. The next day, some of these areas will have developed a redness , while others will be unaffected. The lowest dose to produce just perceptible redness is known as the minimal erythemal dose . This process checks for any unusual sensitivity to UVB and determines the UVB exposure dose for the first treatment. From this short exposure on the first visit, subsequent visits will feature increasing amounts of UVB. This allows the skin to become used to the UVB, in a similar way that one may gradually increase time in the sun on holiday to avoid immediate sunburn.

What Is Narrowband Uvb Treatment

Handheld 311nm Narrowband UVB Lamps For Vitiligo Psoriasis ...

Narrowband UVB is used to treat guttate and plaque psoriasis that is particularly widespread or has not responded to topical treatments. Narrowband UVB treatment may sometimes also be referred to as TL-01 this is the name of the most commonly-used narrowband UVB lamp.

UVB treatment is given in a phototherapy unit usually in a hospital Dermatology department or clinic – by a team of health professionals. A Dermatologist calculates precisely how much UVB light each persons skin should be subjected to, and will increase the exposure over the course of the sessions.

Treatment is usually given two or three times a week, for up to ten weeks. The individual stands in the UVB cabinet for a period of a few seconds to several minutes at a time. Depending on which areas of the body are being treated, they may be advised to cover up certain areas, and leave other areas exposed. You should always ask your Dermatologist or the member of staff administering the UVB treatment if you have any questions about this.

Sunbeds Sunlamps And Natural Sunshine

Using a sunbed at a gym, salon or spa is not the same as having UV therapy in a hospital setting. Firstly, hospital-based UV treatment uses only the specific part of the spectrum that is useful to treat skin conditions. This is not the case on a sunbed, where a much broader spectrum is used. Many sunbeds use mostly, or entirely, UVA light, which is ineffective for treating psoriasis without the added psoralen. Therefore, using sunbeds means taking on the risks of UV exposure, without much of the benefit to psoriasis.

UV treatment in hospital is very carefully controlled a Dermatologist will administer the right amount of exposure for each individual, and monitor the results. If a person uses sunbeds or an at-home lamp in addition to this, it makes it difficult to ensure they are receiving the correct dose for them.

It is also important to follow sun safety advice, even if your psoriasis improves in the sunshine, in order to prevent potential side effects such as skin cancer and premature ageing. Sunburn can actually aggravate psoriasis or trigger a flare, so do use a suncream with a high SPF and re-apply it regularly. Aim to cover up with a hat, t-shirt and sunglasses, and avoid being out in the intense sunlight in the middle of the day.

What Are The Risks/side Effects Of Uv Therapy

  • Exposure to UV light can cause skin damage, premature ageing and increases the risk of skin cancer. However, it is important to note that this is true of natural sunlight as well as artificial UV light. For these reasons, the British Photodermatology Group have issued guidelines as to the total number of UV treatments an individual can have in a lifetime.
  • Some redness of the skin and tanning is likely, but you should let the staff in the department know if you experience any burning sensations .
  • If you are using other treatments, including topical treatments, you should check with your doctor or pharmacist that it is appropriate to keep using it during your UV treatment. This is because some other treatments can make the skin more sensitive to UV light, which may increase the likelihood of side effects if used at the same time.
  • UV treatment can make the skin dry and itchy apply plenty of moisturiser after treatment in order to overcome this. However, if the itching continues or gets worse, do tell the phototherapy staff or your Dermatologist.
  • Oral Psoralen used in PUVA can make some people feel sick. Do tell your Dermatologist if this happens for you, as they may be able to change the type of Psoralen, or prescribe another medication to help you stop feeling sick.

Psoriasis UVB Light Treatment From Home – Vlog #3

In the late 1980s, Philips Lighting of Holland developed just such a lamp, known as the TL-01 or UVB Narrowband lamp. The smaller grey shaded area in FIGURE D shows that UVB-Narrowband lamps have considerably less erythemogenic output than conventional UVB-Broadband lamps. This means that more therapeutic UVB can be delivered before erythema occurs, and since erythema is a risk factor for skin cancer, these new lamps should theoretically be less carcinogenic for the same therapeutic results4,5,6,7. Furthermore, and critical to the success witnessed by home UVB-Narrowband phototherapy, it becomes much more possible that the disease is controlled without ever reaching the erythemogenic threshold9,10, which was always a problem with UVB-Broadband treatments. It is interesting to note that the peak of the UVB-Narrowband curve is about ten times higher than the UVB-Broadband curve, thus the source of the name Narrowband.

More recent studies have confirmed these findings and also determined that UVB-Narrowband has fewer burning incidents and longer remission periods than UVB-Broadband. When compared to PUVA , UVB-Narrowband has significantly fewer side effects and has replaced it in many cases8.

Other diseases such as vitiligo, eczema, mycosis fungoides , and many others have also been successfully treated with UVB-Narrowband, generally for the same reasons as described above for psoriasis.

    When Is Uv Therapy Used

The National Institute for Health and Care Excellence recommends that UVB therapy be offered to people with plaque or guttate psoriasis that cannot be controlled with topical treatments alone. They are likely to have tried a number of different topical treatments before being offered UVB therapy . It recommends that PUVA treatment be considered to treat palmoplantar pustulosis .

If it is found that some areas of psoriasis are slow to respond, do not show a satisfactory response, or are in some hard-to-treat areas , a topical treatment might be prescribed alongside UV therapy. It is important to remember that the effects of UV therapy are temporary in most cases the psoriasis will come back at some point. If psoriasis is returning very quickly after a course of UV therapy, it may be time to move on to another type of treatment.
