How Long To Use Uvb Lamp For Psoriasis

What Size Should I Get

Treating Psoriasis | Portable UVB Light Therapy Device 2019

In general, there are two types of UVB therapy units. First up: Hand-held devices. These units are best if you only have a small surface area affected by the disease. If only 2% of your body needs to be treated or its in isolated spots such as your neck, legs, or hands, a hand-held one is great, says Jessica Kaffenberger, M.D., an associate professor of dermatology at Ohio State University in Columbus. However, if youre treating a larger areasay, your back, butt, trunk, arms, and legs, you will need a much larger unit.

Full-body machines are much more practical if the disease covers a lot of your body surface, says Carolyn Baker, P.A., a dermatology physician assistant at Newburgh Dermatology in Newburgh, NY. But these units might require a little rearranging of the furniture. Some are shaped like a booth with three vertical panels that you step inside others are more horizontal and open at the top. Not all of them are quite so unwieldy: A few fold up flat when youre done. Ask your doctor about options.

    Types Of Uv Light Treatment

UV light treatment comes in three forms with Narrow Band Ultraviolet B Light being the most common these days. NB-UVB systems try to produce a narrow band of light only in the ranges that are effective in treating psoriasis.

NB-UVB is also preferred because it causes side effects in fewer people, is easier to use, and has a wider range of uses in treating skin conditions.

Who Is A Candidate For Uvb

Kernel LED light 311 nm uvb lamp targeted uv phototherapy ...

Both adults and children can benefit from UVB treatment. It is effective in treating psoriasis for at least two-thirds of patients who meet these conditions:

    • Thin plaques
    • Moderate to severe disease
    • Responsive to natural sunlight

A treatment program may include medicines applied to the skin or taken by mouth, as well as UVB. Topical medicines, such as anthralin, coal tar, calcipotriene and tazarotene are effective along with UVB in some people. Using systemic drugs such as methotrexate, biologics and acitretin with UVB may also improve the effectiveness of the treatment.

Will I Get A Tan Using A Home Uvb Phototherapy Device

Some people report that they get a tan and others do not. UVB is known to create more melanocytes in your skin, the cells needed for maximum skin darkening, but UVA light is the primary contributor to tanning. Dosages also play an important role. The SolRx Users Manual provides conservative treatment times. Excessive tanning has not been reported. More likely is some temporary skin reddening if the dosage approaches its maximum. Skin reddening usually fades within a day.

    Treatment Protocols And Follow

Although minimal erythema dose determination and subsequent dose calculation is the formal method of establishing initial dose, this process is often cumbersome and labor intensive. Therefore, most phototherapy centers have now adopted the use of initial dose determination based on patients Fitzpatrick skin type. Increments of dose increase are also determined by skin type in addition to response to previous treatments. The underlying principle is to increase the UVB dose gradually until the MED is reached and then try to maintain the UVB dose just below the MED . This approach eliminates the need for formal MED testing in most patients. It is also simpler and more efficient than methods using incremental dosages calculated as a certain percentage of the previous dosage.

A standard protocol for UVB phototherapy is treatment three times per week with a minimum of 24 hours between sessions. Treatment every other day is effective for most patients. Patients should be instructed to dose phototherapy as per treatment protocol and use proper technique as taught during outpatient phototherapy and home phototherapy orientation. Moisturizer should be immediately applied following treatment to prevent excessive dryness and subsequent itching.

How Much Room Do I Need For A Solrx 1000

The SolRx 1000-Series Full Body Panel is designed to take up a minimum amount of space in your home. They are only 3½ thick by 29 wide by 72 high and mount flat up against a wall, or in a corner, with the bottom resting on the floor and the top fastened to the wall in two places. Casters are not practical because they must have a wide wheelbase to keep the unit from tipping over, and therefore greatly increasing the floor space needed. Casters also increase the distance between the bottom of the bulbs and the floor, making a platform necessary for lower leg treatment. Over 25 years of experience with the 1000-Series has shown that once the device is mounted, it is out of the way and there is little need to have it moved. The authors personal 1000-Series unit was in the same place for over 20 years, until just recently being replaced by an E-Series 1M+4A.

    How Safe Is Ultraviolet Phototherapy

    Treat Psoriasis with Phototherapy

As with natural sunlight, repeated exposure to ultraviolet light can cause premature aging of the skin and an increased risk of skin cancer. However, when only UVB is used and UVA is excluded, many decades of medical use have proven that these are only minor concerns. Indeed, UVB phototherapy is drug-free and safe for children and women that are pregnant.

When these relatively minor risks of UVB phototherapy are weighed against the risks of other treatment options, often involving strong prescription drugs or even injections, UVB phototherapy is usually found to be the best treatment option, or at least the treatment option that should be tried after topical drugs such as steroids and dovonex have proven minimally effective.

Many governments issue a formulary for each biologic drug that says phototherapy must be attempted before the biologic can be prescribed, but unfortunately often with the caveat , which too often pushes patients into the more risky, expensive, and unnecessary biologic drug.

Other practical safety considerations with phototherapy are that all persons exposed to the UV light must wear eye protection, with patients wearing the UV-blocking goggles supplied with the SolRx device, and with males covering both their penis and scrotum using a sock, unless that area is affected.

How Long Does It Take To Get Results

Typically some remission is evident after only a few weeks, while more advanced clearing requires two to six months and sometimes up to a year for the worst cases. Once the skin has significantly cleared , treatment times and frequency can usually be reduced and the skin maintained in its healthy condition for many decades.

A bonus is that every UVB treatment makes large amounts of Vitamin D in the skin for general health benefits as well.

    Broadband Ultraviolet B Light Therapy

Broadband ultraviolet B light therapy is an older form of phototherapy than NB-UVB. The two treatments are similar.

However, BB-UVB lamps and light bulbs emit wavelengths of light between 270 and 390 nm.

As with NB-UVB, your starting dose will depend on your skin type.

According to a small 1981 study, 90 percent of people had clear skin after having sessions three times a week and an average of 23.2 treatments.

One hundred percent of people had clear skin after having sessions five times a week and an average of 27 treatments.

BB-UVB is considered less effective than NB-UVB and is more likely to cause side effects. It should be reserved for instances where NB-UVB isnt a treatment option.

BB-UVB is most effective for plaque psoriasis, although it can also be used for guttate psoriasis.

It can be prescribed as a monotherapy or alongside retinoid acitretin . In combination therapy, the skin clears up faster, and lower doses of UVB can be used.

Are The Solrx Models With More Bulbs Physically Larger Devices

No. For a given device family, all models use the same steel frame components and differ only in the number of bulbs installed. For example, the eight-bulb 1780 uses the same main frame as a four-bulb 1740, but the bulbs are packed in much tighter to increase the device irradiance and reduce treatment times. Devices with more bulbs will also have more ballasts, and be a bit more heavy.

Just like the treatments you would receive at your doctors office, you will likely need to use these machines in small increments, say, two 5-minute sessions, three times a week, Dr. Kaffenberger says. This isnt usually a big time commitment and is definitely less of a time commitment than applying topical therapies, she says. Ive found that patients are likelier to do UVB therapy when the unit is in their home than if they have to travel to my office for it.

    Does Normal Sunlight Help

Regular sunlight has been used for centuries to treat psoriasis and other skin conditions. Some people suggest using a sunblock on unaffected areas then covering the affected areas about 10 or 15 minutes after youve been in the sun.

You just have to be careful since sunburn can actually have the opposite effect on psoriasis and actually make it worse. Short exposure times are key.

Make sure you talk to your doctor before you decide to try sun exposure just to make sure its right for you.

Uv Light Therapy For Moderate To Severe Cases

Casey Gallagher, MD, is board-certified in dermatology. He is a clinical professor at the University of Colorado in Denver, and co-founder and practicing dermatologist at the Boulder Valley Center for Dermatology in Colorado.

, also known as UV light therapy, is one of the most effective treatments for psoriasis. It involves exposing the skin to controlled amounts of ultraviolet radiation similar to that from the sun. It is usually prescribed with other treatments when conservative therapies fail to provide relief and is especially useful for moderate to severe psoriasis covering large portions of the body.

Phototherapy is performed in a dermatologist’s office under controlled conditions. Side effects tend to be mild. In addition to psoriasis, phototherapy may also be used to treat eczema, vitiligo, lichen planus, and cutaneous T-cell lymphoma.

Narrowband Uvb Phototherapy For The Treatment Of Psoriasis

UVB Lamp for Treatment Psoriasis Vitiligo Eczema Atopic Dermatits HOME Phototherapy

With approximately 150,000 newly diagnosed cases of psoriasis each year, this chronic skin disease affects approximately 2% of the US population and causes considerable morbidity. First-stage treatment of patients with moderate- to-severe psoriasis often starts with topical agents and then progresses to phototherapy for more extensive disease. Extensive disease that cannot be controlled adequately with UVB treatment is then treated with second-stage agents such as psoralen plus UVA , methotrexate, cyclosporine, or other oral medications. In the past, PUVA and oral agents such as methotrexate have been viewed as effective outpatient treatments. However, concerns regarding PUVA- induced cutaneous carcinoma and melanoma have restricted its use.

Furthermore, long-term treatment with oral agents such as methotrexate have a number of systemic toxic effects. As a result, recent efforts to treat psoriasis have become geared toward a particular type of phototherapy known as narrowband UVB .

How Effective Is Uv Light Treatment For Psoriasis

UV light treatment has been very effective in a wide range of patients. Studies show that 60-75% of people that try phototherapy achieve at least a 75% improvement in around 20 sessions! This success rate has led to quite a few hand held NB-UVB units being made for home use.

These handheld units are great for treating scalp psoriasis as well as other areas of the body.

Excimer laser treatment is another light therapy that is showing a lot of promise. Its basically a high energy NB-UVB laser that can be very effective in treating areas with heavy plaques.

Since these areas need more light to effectively treat than other areas the laser is able to use more energy in these areas to get rid of psoriasis. The downside is that it isnt very effective over large areas of the body.

Remission from laser treatment is generally longer than with NB-UVB treatment but cannot be done at home.

How Often Are Treatments Taken And How Long Are The Treatment Times

Recommendations for treatment time and frequency are provided in the psoriasis, vitiligo, or eczema Exposure Guideline Table in the devices Users Manual. In all cases, the patient always starts with a sufficiently low treatment time to ensure they will not get a skin-burn, which is typically just seconds long per treatment area. Then, if treatments are taken on a regular basis per the treatment schedule, treatment times are gradually increased possibly up to several minutes long when the skin might show the onset of a very mild burn, which represents the maximum dose. The results of the last treatment and number of days since that last treatment are used to determine the treatment time for the current treatment. The patient continues on this basis until the skin is significantly clear, which can take 40 or more treatments over several months or more. Then, for maintenance, treatment times and frequency can be reduced as the patient finds a balance between minimizing UV exposure and the condition of their skin. Maintenance treatments can continue like this for many decades, essentially solving the problem naturally and drug-free. Many thousands of home UVB-Narrowband phototherapy patients have proven this.

For vitiligo, treatments are usually taken twice per week, never on consecutive days. Treatment times are usually less than those for psoriasis.
