Vitamin D And Psoriasis Linkage

In psoriasis, vitamin D is involved in the maintenance of cutaneous barrier homeostasis. Several studies identified an association between polymorphisms of vitamin D receptor and psoriasis susceptibility . Richetta et al., have found that the A-1012G promoter polymorphism of the VDR gene is associated with psoriasis risk through a lower expression of VDR mRNA, favoring conditions that may alter cutaneous barrier and the development of psoriatic lesions . In addition, in psoriatic skin a decreased expression of VDR and reduced tight-junction proteins is associated . Tight junctions are fundamental to regulate adhesion and permeability of keratinocytes, and to polarize cutaneous cell differentiation, to regulate extracellular calcium gradient, interacting with nuclear and cytoplasmic proteins and influencing the regulation of specific genes involved in keratinocytes differentiation and proliferation . Different studies have focused on the possible role of low vitamin D status in the pathogenesis of psoriasis .

Clinical Trials Evaluating Vitamin D In Patients With Psoriasis

Disphanuarat et al20 compared the efficacy of vitamin D2 60,000 IU administered orally every other week for six months to placebo in patients with psoriasis. Psoriasis Area and Severity Index scores were assessed at three and six months, as well as laboratory evaluations of serum levels of 25D, calcium, phosphate, parathyroid hormone, and C-reactive protein . After just three months of vitamin D supplementation, PASI scores were significantly improved compared to placebo. While the PASI scores at six months were not statistically significant, there was a trend toward more improvement in the supplementation group. Moreover, the researchers found an inverse relationship between the severity of psoriasis and serum vitamin D levels.

In a review article by Zuccotti et al,21 researchers evaluated the effects of several nutritional strategies for patients with psoriasis, including hypocaloric diet, vitamin D, fish oil, selenium, and zinc supplementation. Researchers concluded that vitamin D supplementation in patients with psoriasis who are deficit in the vitamin might aid in preventing psoriasis-related comorbidities. The authors also noted a hypothetical association between vitamin D deficiency and obesity and psoriasis.21

Ultimately, these data suggest that oral vitamin D is both effective and safe for the management of psoriasis.

Are There Side Effects Of Vitamin D

Getting too much vitamin D can have negative side effects. Youre unlikely to get an unsafe amount of the nutrient from food or sunlight. However, you could get too much from supplemental sources.

High levels of vitamin D can cause damage to your kidneys, blood vessels, and heart. In extreme cases, they can cause death. More often, excess vitamin D can lead to high levels of calcium in the blood due to increased calcium absorption. High calcium can cause side effects such as nausea, vomiting, dehydration, confusion, weakness, and kidney stones.

For safety, consult with your health care provider before taking over-the-counter vitamin D supplements. They may check your vitamin D levels, via blood work, prior to recommending a supplement.

Vitamin K2 And Vitamin D

Vitamin D toxicity, albeit rare, can cause hypercalcemia and, therefore, buildup calcium in the blood stream, leading to deposition in the arterial walls and kidneys, and can also exacerbate osteoporosis.11 Mitigation strategies lie in the biochemistry of the vitamin D pathways and lead to understanding a crucial role of vitamin K2. Studies suggest that vitamin D supplementation that led to hypercalcemia was linked to vitamin K2 deficiency, as this nutrient activates osteocalcin through carboxylation and can lead to calcium deposition in the bones, whereas nonactivation inhibits bone reabsorption.11 As vitamin D levels rise, more K2 is required to balance these biochemical reactions to remineralize bone rather than arterial walls or the kidneys.19

The vitamin D, like other fat-soluble vitamins , is absorbed incorporated in mixed micelles from the intestine into the enterocytes by non-saturable passive diffusion. Subsequently, the vitamin D is transported in the chylomicrons via lymph to the circulation . The more polar metabolite 25D is absorbed better and faster than vitamin D because it is also taken up directly from the proximal jejunum into the portal vein . There are few data on its availability from natural sources. The absorption of vitamin D from supplements may differ depending on the used vehicle substance, such as oils, powders, ethanol . For example, it has been reported that the bioavailability of vitamin D from fortified hard cheese is equivalent to supplements and that vitamin D bioavailability is not influenced by the fat content of the fortified milk .

Vitamin D Treatments Target Psoriasis

Study Helps Explain How Ultraviolet Light Therapy and Vitamin D Creams Treat Psoriasis

May 12, 2011 Ultraviolet light therapy and vitamin D creams are widely prescribed treatments for psoriasis, and now a new study may help explain why they work for so many patients.

Researchers say the vitamin D-based treatments increase the binding of a peptide called cathelicidin to DNA, which, in turn, inhibits the inflammatory response that triggers psoriasis.

The finding may one day lead to better treatments for the painful skin condition that specifically target cathelicidin, study researcher Jurgen Schauber, MD, of Ludwig-Maximillian University in Munich, Germany, tells WebMD.

âWe were able to identify a novel, pro-inflammatory signaling pathway which helps us understand why treatments work,â he says.

What Factors Trigger Psoriasis

Certain factors may trigger or aggravate psoriasis symptoms. It is preferable to identify these factors so that they can be avoided to the greatest extent possible.

These precipitating factors include:

  • Topical calcineurin inhibitors :
  • The U.S. Food and Drug Administration has not approved them for psoriasis, and they have not been proven effective for plaque psoriasis, but they may be used off label for psoriasis that affects the face and intertriginous areas, where they may be effective and allow people to avoid chronic corticosteroid use.
  • Although no definitive cause and effect relationship has been established, the FDA issued a warning about a possible link between these medications and cases of lymphoma and skin cancer in children and adults.
  • It is known to be beneficial and is used in the treatment of moderate-to-severe psoriasis or severe psoriasis.
  • Natural sunlight , ultraviolet B radiation, and psoralen plus ultraviolet A radiation are more common options.
  • Treatment with a high-energy excimer laser is effective and safe for treating localized psoriasis while requiring fewer office visits and sparing uninvolved skin.
  • To improve efficacy, phototherapy can be combined with other treatments.
  • Phototherapy may increase the risk of skin cancer.
  • Systemic therapy:
  • People with moderate-to-severe, severe or treatment-resistant psoriasis, as well as those with psoriatic arthritis, may require this treatment.
  • Nonsteroidal anti-inflammatory drugs :

    Vitamin D For Psoriasis

Plenty of people with psoriasis use different combinations of medications to treat the condition. For some, the psoriasis symptoms may improve significantly just by increasing their vitamin D.

Vitamin D, which your body can make when exposed to sunlight, has many health benefits that may help treat psoriasis. Vitamin D deficiencies have been linked to psoriasis. While a deficiency doesnt seem to cause psoriasis outright, it may impair the bodys ability to keep the skin healthy. This may increase flares. When taken in healthy doses, vitamin D can help treat psoriasis. Heres how.

Vitamin D possesses different health benefits that can help treat several types of psoriasis, including scalp psoriasis.

A study from 2011 found that vitamin D can strengthen the immune system. Because psoriasis is an autoimmune response, this effect could help treat the condition internally.

Topical oils and ointments for psoriasis containing vitamin D can also help treat flares. Vitamin D can slow the growth of new cells, so vitamin D oil applied directly to the flare can help thin the plaque.

There are different forms of vitamin D that you can use to help treat your psoriasis.
