Betamethasone Valerate Foam For Treatment Of Nonscalppsoriasis

New spray on foam treatment can clear psoriasis in less than a month

Background: Although betamethasone valerate foam, 0.12% is approved by the Food and Drug Administration for the treatment of corticosteroid-responsive scalp dermatoses, no data are available for its use on nonscalp psoriasis. Objective: We evaluated the safety and efficacy of BMV foam in treating psoriatic lesions at nonscalp sites. Methods: We conducted a randomized, double-blind, placebo-controlled, paired-comparison, split-body study of 40 patients with mild to moderate plaque-type psoriasis. Patients applied BMV foam and placebo foam twice daily for 12 weeks. Results: At the end of the treatment period, 70% of patients had greater than 50% improvement of lesions on their active foam-treated side compared with 24% of patients with similar improvement on their placebo foam-treated side. Adverse effects were limited to temporary stinging, burning, or itching in several patients. Three patients withdrew because of stinging or itching. Conclusions: The results indicate that BMV foam is effective against nonscalp psoriasis. Twice-daily applications are well tolerated, compliance exceeds 90%, cosmetic characteristics are acceptable, and the medication may reduce the need for multiple prescriptions.

Effects On Endocrine System

Hypothalamic-Pituitary-Adrenal Axis Suppression

Systemic absorption of topical corticosteroids can cause reversible hypothalamic-pituitary-adrenal axis suppression with the potential for clinical glucocorticosteroid insufficiency. This may occur during treatment or upon withdrawal of treatment. Factors that predispose a patient to HPA axis suppression include the use of high-potency steroids, large treatment surface areas, prolonged use, use of occlusive dressings, altered skin barrier, liver failure, and young age.

Evaluation for HPA axis suppression may be done by using the adrenocorticotropic hormone stimulation test. If HPA axis suppression is documented, gradually withdraw Enstilar Foam, reduce the frequency of application, or substitute with a less potent corticosteroid.

Systemic effects of topical corticosteroids may also include Cushing’s syndrome, hyperglycemia, and glucosuria.

Pediatric patients may be more susceptible to systemic toxicity due to their larger skin surface to body mass ratios .

Use of more than one corticosteroid-containing product at the same time may increase the total systemic corticosteroid exposure.

If You Use Too Much Enstilar

If you think that you have used too much Enstilar®, you may need urgent medical attention.

You should immediately:

  • phone the Poisons Information Centre, or
  • contact your doctor, or
  • go to the Emergency Department at your nearest hospital.

You should do this even if there are no signs of discomfort or poisoning.

How Long To Use Enstilar

  • Each treatment course would normally last for up to four weeks.
  • Your doctor may decide to stop treatment with Enstilar® or require you to use it twice weekly once your psoriasis has improved.

How Long Should You Use It

According to the National Library of Medicine, a person can use clobetasol propionate cream for up to 2 weeks at a time. If the psoriasis is under control within a shorter period, they should stop using the product then.

The duration and frequency of use may vary among different forms of clobetasol propionate. A person should talk with their doctor about how to use their prescribed medication safely.

Clobetasol propionate and other potent corticosteroids are a common treatment for psoriasis. Research suggests that clobetasol propionate generally works well to clear or nearly clear psoriasis patches within 24 weeks.

In an older study , researchers found clobetasol propionate to be very effective in treating psoriasis. However, they note that a doctor should work with a person to determine their preferred application method and maximize the likelihood of adherence to the treatment regimen. Following the care routine according to the doctors advice should lead to a better outcome.

Clobetasol propionate has some associated risks and side effects, including:

  • causing discoloration at the site of application, although the risk is low
  • suppressing the hypothalamic-pituitary-adrenal axis, meaning that a person may have a reduced immune system response or feel fatigued, anxious, or depressed

Do Complementary And Alternative Approaches Work For Plaque Psoriasis

Plenty of people with psoriasis turn to non-traditional therapies for relief. Responding to a 2019 patient survey described in the Journal of the American Academy of Dermatology, 41% of patients reported using alternative treatments in place of traditional medicine in the last five years, while 50% said that they have added complementary remedies to their treatment regimen.
