How To Take Methotrexate
Methotrexate comes in many forms. You may be prescribed a pill or liquid. Some patients inject methotrexate at home. Each form has benefits. For example, injecting methotrexate can reduce side effects such as nausea and vomiting.
Before taking methotrexate for the first time, be sure to carefully read the instructions. If the instructions are confusing or differ from what your dermatologist said, call your dermatologist before taking methotrexate. Some patients stay on methotrexate for a long time. Others stop taking methotrexate once their psoriasis is under control and restart when psoriasis flares.
Follow recommended dosage
Most people take methotrexate once a week. Taking it more often than your dermatologist prescribes can cause serious health problems.
Common Questions About Methotrexate
Methotrexate calms your immune system, to help stop it attacking your body’s cells. This helps reduce the inflammation that causes swollen and stiff joints in rheumatoid arthritis, thickened skin in psoriasis or damage to your bowel in Crohn’s disease.
Methotrexate is not a painkiller. However, by reducing the inflammation caused by the condition, you may notice a reduction in pain as your symptoms improve.
When used in cancer treatments, methotrexate stops cancer cells reproducing. This helps prevent them growing and spreading in your body.
Once your dose of methotrexate has been increased to a full dose, it may take up to 12 weeks before you notice any benefits.
It’s important to keep taking methotrexate. The medicine is working, even if you do not feel any different for a few months.
If methotrexate works for you, you may need to take it for several years to control your symptoms.
Methotrexate may increase the risk of problems with your liver or lungs, especially if you take a high dose.
However, your doctor will monitor you very closely while you’re taking methotrexate. Any concerns or problems will be found during routine check-ups.
Speak to your doctor or specialist nurse if you are worried about taking methotrexate.
There is no evidence that methotrexate increases the risk of developing cervical cancer.
When you start your treatment, your hospital consultant will usually give you the first few prescriptions for methotrexate. You will get 1 week’s supply at a time.
How Coming Off Methotrexate Doubles Antibody Levels After Covid
People with conditions like psoriasis and rheumatoid arthritis who took a two-week break from the immune-suppressing drug methotrexate saw their antibody levels double up to 12 weeks after their third covid-19 vaccine, compared with people who took the treatment as normal People who take a widely used medicine to suppress their overactive immune system may respond better to covid-19 vaccines if they come off the drug for two weeks after being immunised.
Having a two-week break from the once-a-week drug methotrexate taken by more than 1 million people in the UK alone gave people with conditions such as rheumatoid arthritis and psoriasis double the antibody response to their third vaccine, compared with carrying on with the treatment as normal, a study found.
The break was linked to a temporary worsening of symptoms, but this was generally modest and manageable with other medicines, such as painkillers.
Autoimmune diseases are caused by cells of the immune system attacking various parts of the body. They are treated with medicines that rein in the immune system, but these tend to leave people with a worse response to both infections and vaccines.
Amid the coronavirus pandemic, some doctors groups in the US recommended people stop taking the medicine for one week after receiving a covid-19 vaccine, but their UK counterparts advised against this.

Causes Of Psoriatic Arthritis
Almost 1 in 3 people with psoriasis also have psoriatic arthritis.
It tends to develop 5 to 10 years after psoriasis is diagnosed, although some people may have problems with their joints before they notice any skin-related symptoms.
Like psoriasis, psoriatic arthritis is thought to happen as a result of the immune system mistakenly attacking healthy tissue.
But it’s not clear why some people with psoriasis develop psoriatic arthritis and others do not.
What Was The West Virginia V Epa Case About
This case has its roots in a complicated legal fight over who has authority to regulate greenhouse gas emissions from power plants. Electricity production is the second-largest source of carbon emissions behind transport in the US.
In the 1960s, Congress passed the Clean Air Act, giving the EPA authority to enforce regulations to improve air quality. In 2015, the Obama administrations Clean Power Plan set guidelines for states around carbon dioxide emissions from power plants. Twenty-four states sued, arguing that the plan overstepped the authority granted to the EPA by the Clean Air Act.
In 2019, before the Supreme Court ruled on that issue, the Trump administration replaced the Clean Power Plan with the Affordable Clean Energy rule, which was less strict. A group of states sued, and a federal circuit court blocked the rule. That left the incoming Biden administration with a clean slate to set rules on greenhouse gas emissions.
It didnt last long. A number of coal companies and coal-producing states, led by West Virginia, petitioned the Supreme Court to reconsider the circuit courts decision.
Many legal observers were surprised when the Supreme Court agreed to hear the case, since neither the Clean Power Plan nor Affordable Clean Energy rule had actually gone into effect.

Ask your doctor about changing to a different medicine before you stop using contraception.
Methotrexate may sometimes make you feel tired, or dizzy and light-headed.
If you are feeling tired or dizzy, do not drive, ride a bike or use tools or machinery until you feel better.
Myth: Methotrexate Is Only For The Worst Cases Of Rheumatoid Arthritis
Not at all. American College of Rheumatology guidelines call for starting treatment with methotrexate or another DMARD as soon as a diagnosis of RA is made, even if your disease activity is considered low.
Treating early and aggressively can keep the worst from ever happening by preventing joint damage. In a large study published in 2015, McGill University researchers found that patients who spent more time on methotrexate or another DMARD during their first year after diagnosis were more likely to delay or avoid the need for joint replacement.

Efficacy Of Mtx In Psoriasis And Psa
To open the debate on MTX use, S.M. Grieb discussed personal experiences from the GRAPPA PRP. Prior to the 2019 GRAPPA annual meeting, PRP were asked to respond to questions regarding their personal experience using MTX. Of the PRP, 11 of 13 responded, with 10 sharing their experiences, and 1 indicating no MTX use. Of those with experience using MTX, the majority used the tablet form, sometimes as monotherapy and sometimes in conjunction with additional medicines. Only 3 PRP had experience with the injectable form. Most of the PRP indicated that MTX did little to help their disease. The benefits they experienced were largely in the skin, while there was only a limited effect reported on arthritis. For the 3 PRP who responded well to the medication, the effect on their health and well-being was great. One PRP stated, At first it was a revelation. Relatively quickly I was relieved of the feeling that I was moving my body through the weight of deep water, and movement became easier again, less painful. The psoriasis relief followed. However, for 2 of the 3 who found relief using MTX, it was unfortunately not sustained.

If you’ve been a reader of this blog, you’ll know that Methotrexate is a go-to drug for rheumatologists .
It’s considered safe, in the hands of rheumatologists and with monitoring.
It’s considered the cornerstone of effective treatment for rheumatoid arthritis, it’s a standard medication for psoriatic arthritis, and we often use it for it’s “steroid-sparing” effect .
But, not everyone can tolerate it.
It’s important patients know this. There are alternatives and I don’t expect my patients to accept side effects which are significant to them, or to me.

When To Get Medical Advice
See a GP if you have persistent pain, swelling or stiffness in your joints even if you have not been diagnosed with psoriasis.
If you’ve been diagnosed with psoriasis, you should have check-ups at least once a year to monitor your condition. Make sure you let the doctor know if you’re experiencing any problems with your joints.
Work And Psoriatic Arthritis
Having psoriatic arthritis may make some aspects of working life more challenging. But, if youre on the right treatment, its certainly possible to continue working.
Help and support is available, and you have rights and options.
The Government scheme Access to Work is a grant that can pay for equipment to help you with activities such as answering the phone, going to meetings, and getting to and from work.
The 2010 Equality Act, and the Disability Discrimination Act in Northern Ireland makes it unlawful for employers to treat anyone with a disability less favourably than anyone else. Psoriatic arthritis can be classed as a disability if its making every-day tasks difficult.
Your employer may need to make adjustments to your working environment, so you can do your job comfortably and safely.
You might be able to change some aspects of your job or working arrangements, or train for a different role.
In order to get the support youre entitled to, youll need to tell your employer about your condition. Your manager or HR department might be a good place to start.
Other available support might include:
- your workplace occupational health department, if there is one
- an occupational therapist. You could be referred to one by your GP or you could see one privately
- disability employment advisors, or other staff, at your local JobCentre Plus
- a Citizens Advice bureau particularly if you feel youre not getting the support youre entitled to.

Should You Pause Methotrexate After Getting A Covid
Initial studies have found that some patients on methotrexate have a decreased response to the COVID-19 vaccine. While more research is needed, some experts and medical organizations have recommended that patients hold methotrexate after getting the vaccine others have not.
- The American College of Rheumatologys COVID-19 Vaccine guidance recommends that patients with well-controlled disease hold methotrexate for one week after each COVID-19 vaccine dose or for two weeks after a single-dose vaccine .
- The National Psoriasis Foundations COVID-19 Task Force Guidance recommends that patients who are to receive a mRNA-based COVID-19 vaccine continue their biologic or oral therapies for psoriasis and/or psoriatic arthritis in most cases. However, it also says that for people who are 60 or older or have comorbidities linked with poor COVID-19 outcomes and have well-controlled disease could consider holding methotrexate for two weeks after getting the one-dose Johnson & Johnson vaccine.
Experts who recommend pausing methotrexate temporarily after getting a COVID-19 vaccine suggest this because it may help the vaccine protect you as much as possible.
Experts who recommend continuing to take methotrexate after getting a COVID-19 vaccine may have concerns that patients could flare if they stop taking it, or are not as worried about the potential decrease in vaccine effectiveness.

How Methotrexate Affects The Immune System
Before methotrexate was widely used to treat rheumatologic conditions, it was originally approved as a form of chemotherapy for cancer. But it works in the body differently when used in much lower doses to treat inflammatory rheumatic conditions like rheumatoid arthritis than higher doses used for cancer treatment.
For rheumatic conditions, methotrexate is taken as pills or a subcutaneous injection, usually once per week.
Methotrexate is considered an antiproliferative medication, which means it stops cells from replicating. It reduces the activity of several types of proteins and cells involved in immune system function, including T cells and B cells.
This description from Dr. Kim helps explain how methotrexate affects the immune system:
Lets say your immune system is an army fighting a virus enemy. It needs to be able to quickly increase the number of its troops. For a typical virus, the immune system can expand its army by a million-fold. But methotrexate limits your immune systems ability to scale its troops. Instead of expanding by a million, maybe it can only increase by a thousand. This inability to increase your troops makes you less able to fight off an infection.

Methotrexate And Folic Acid Dose Recommendations
Because of its effectiveness and established safety profile, most patients who have RA take methotrexate at some point for treatment, usually by mouth. The typical dose starts at about 5 mg to 10 mg per week. However, in cases where high-dose oral methotrexate treatment is needed, the dose can be increased to about 25 mg per week, or even higher if necessary.
For patients in whom treatment based side effects become an issue, the proper dose of methotrexate can also be given as an injection under the skin. Sometimes, simply switching the dose delivery method from an oral methotrexate tablet to a methotrexate injection is enough to help. Splitting the dose into two daily doses instead of one single dose may help as well, or you can ask your doctor about anti-nausea medications.
Also, as the University of Michigan reports that methotrexate stops the growth of rapidly dividing cells, such as embryonic, fetal, and early placenta cells. No dose of methotrexate is safe during pregnancy. In fact, doctors often use this drug as a treatment for ectopic pregnancy, a potentially dangerous condition which involves a fertilized egg attaching itself to the fallopian tubes versus the uterus, essentially terminating the fetus.
As far as folic acid is concerned, doses in the range of 5 to 10 mg per week have been shown to reduce methotrexate toxicity. Folinic acid, a healthy form of folate which doesnt require any type of enzyme conversion, can also be found in some natural foods.
What Monitoring Will I Need
Your doctor will order regular lab tests to check your response to methotrexate and monitor for toxicity. You will need to have your blood counts â white blood cells, red blood cells and platelets â and liver function checked regularly. Your doctor may order additional tests depending on the results.

What Side Effects Can Methotrexate Cause
The most common side effects are:
- Vomiting
- Mouth sores or redness and swelling in the mouth
- Fatigue
- Tiredness, headache, or brain fog
- Sun sensitivity
You may have more serious side effects, including less ability to fight off infections or liver, lung, or kidney issues. Your doctor will probably do a blood test every few months to make sure methotrexate is not damaging these organs.
Talk to your doctor or dermatologist right away about any side effect you notice. Most can be controlled with a tweak to your treatment plan. A folic acid supplement may also help.
If side effects still bother you, ask your doctor to prescribe another treatment. There are now many treatments that work well for psoriasis. This means you donât have to live with medications that make you feel bad.
Your doctor should try different treatments until they find one that clears your psoriasis with few or no side effects.
