These agents were found in the sequence of the discovery that oral vitamin D had a therapeutic effect on psoriatic plaques .
While vitamin D has mainly antiproliferative effects, corticosteroids have mainly anti-inflammatory effects.
The vitamin D analogues correct epidermal hyperproliferation, abnormal angiogenesis, and keratinization and induces apoptosis in inflammatory cells by acting through vitamin D receptors present on keratinocytes and lymphocytes. They also modulate the decrease of IL-1 and IL-6 levels, the reducing of CD45RO and C8+ T cells. They inhibit the epithelial cell growth by increasing transforming growth factor-1 and –2 levels . Many of these effects protect skin from the cutaneous atrophy caused by corticosteroids .
Vitamin D analogues and corticosteroids are the combining topical agents of choice in psoriasis showing a superior efficacy when compared with monotherapy .
The most common side effects are skin irritation, dryness, peeling, erythema, and edema, which can occur in up to 35% of the patients. Adverse effects will diminish along the time.
At the moment, three vitamin D analogues are approved for the treatment of psoriasis: calcitriol, calcipotriol, and tacalcitol.
The corticosteroids effects can be diminished by administrating calcipotriol 0.03% once daily in the morning plus betamethasone valerate once daily in the evening .
Your Body Gets Resistant
You may feel good with a new treatment, only to have your symptoms come back weeks, months, or even years later. This happens when your body makes antibodies against the medicine, chemicals that mistakenly attack the drug as a harmful invader. This can be especially common with biologics. The impaired barrier function in psoriatic skin facilitates the cutaneous penetration of the topical corticosteroid independently from its potency. The concomitant vasodilatation in psoriatic vessels increases the possibility of topical corticosteroids to reach the systemic vessels. A large extent of body surface and long-term use of topical corticosteroids may conduct to a higher concentration of corticosteroids in the blood, leading to systemic side effects. The risk of systemic side effects associated with chronic topical corticosteroid use increases with high-potency formulations.
Myopathy and muscle atrophy are also possible adverse effects of glucocorticoid treatment. It was found that proteolysis is augmented in myocytes, due to a glucocorticoid-mediated increase in the transcription of genes-encoding proteins linked to the ubiquitin-proteasome pathway .

Who Can Use It
Not everyone can use every medication. Certain people may have ailments or are taking other treatments that increase the chance of experiencing adverse reactions. You should ask a healthcare professional about other treatments if any of the following apply to you:
- you have had an allergic reaction to corticosteroids in the past
- you are currently taking any other steroid treatment
- your skin condition is on the face, genitals or includes open wounds
- your skin condition is the result of a bacterial or viral infection

Flare Ups: Common Psoriasis Triggers

It is not likely other drugs you take orally or inject will have an effect on topically applied hydrocortisone. But many drugs can interact with each other. Tell your doctor all prescription and over-the-counter medications and supplements you use. During pregnancy, hydrocortisone should be used only when prescribed. Infants born to mothers who have been using this medication for an extended period of time may have hormone problems.
Steroid Creams Or Ointments
Steroid creams or ointments are used to treat mild to moderate psoriasis in most areas of the body. The treatment works by reducing inflammation. This slows the production of skin cells and reduces itching.
Topical corticosteroids range in strength from mild to very strong. Only use them when recommended by your doctor.

Instructions:
- remember- always wash your hands before and after applying treatment
- you should rub a thin layer of cream/ointment into the affected areas of skin until it has been completely absorbed
- you should apply treatment once or twice daily until you have completed the course – usually 7 days
- do not cover the affected area with a bandage or tight clothing – this can increase the chance of side effects
If you forget to apply treatment, do not apply extra to compensate for the missed dose – this will make it more likely for you to experience bad reactions. Instead, wait until the time for your next dose and continue as normal.
If you find your eczema or psoriasis has not improved after a week of using Hydrocortisone, you should stop treatment and contact your doctor for further medical advice.

Hydrocortisone has topical anti-inflammatory activity of value in the treatment of irritant dermatitis, contact allergic dermatitis, insect bite reactions and mild to moderate eczema.
Posology
Use sparingly over a small area once/twice a day for a maximum period of one week. If the condition has not improved, or worsens, consult your doctor.
This product should not be recommended for use on children under 10 years of age without medical advice.

Steroid Creams Can Help With Skin Inflammation But Are Not A Cure

Is it true that using steroid creams can thin the skin to the point that the skin is damaged? I have never heard of this and have used hydrocortisone and others for skin irritations on my five-year-old son for nearly two years. How much is too much?
Answer:If you have to use hydrocortisone on your son for longer than two weeks at a time without improvement, you should have him evaluated by a dermatologist.
Typically, steroid creams are used to reduce skin inflammation caused by conditions such as psoriasis and eczema. They are the cornerstone and first-line treatment of these and many other skin diseases. Steroid creams have been used for the past 50 years, but are not a cure because they have no effect on the underlying cause of inflammation. They can, however, be very effective in helping to control skin flare-ups and relieve symptoms, such as itching and irritation. Also, they help reduce the likelihood of infection that may occur as a result of scratching and trauma to the skin.
Commonly used topical steroids include hydrocortisone, triamcinolone, fluocinonide, and clobetasol. They work by preventing cells from producing inflammation-causing chemicals that are released in response to a variety of different triggers.
In your son’s case, a dermatologist may want to perform a skin biopsy or recommend different topical steroids or other anti-inflammatory topical medications.

What Precautions Should I Take
You must not exceed 7 days of continuous use of this medicine.
Once you have applied cream/ointment, do not cover with dressings or tight clothing.
Please note that the ingredients chlorocresol and cetostearyl alcohol may cause local skin reactions .
You should also avoid applying the cream/ointment near the eyes. If it gets in the eyes, wash out immediately and seek medical attention. You should also seek help if you experience any visual disturbances of any kind while using this cream/ointment.
You should avoid going near naked flames when using this treatment – it is flammable.

New Combination Treatment Of Topical Corticosteroids

As an example, calcipotriol is just compatible with tar gel and halobetasol propionate preparations and it has a superior effect when combined with halobetasol ointment. Halobetasol decreased the irritant dermatitis caused by calcipotriol . Also ammonium lactate is compatible with hydrocortisone valerate and halobetasol propionate, and it has been shown to protect against skin atrophy .
Salicylic acid, vitamin D analogues and retinoids, with different mechanisms of action, are usually combined with topical corticosteroids. Concerning polytherapy versus fixed-dose combinations, the last one requires less frequent applications and has a higher adherence from the patients.
